Different clinical and pathological profiles of sudden cardiac death victims caused by coronary artery dissection or myocardial infarction with non-obstructed coronary arteries

Abstract Background Spontaneous coronary artery dissection (SCAD) is a potential cause of acute syndrome and sudden cardiac death (SCD). Coronary fibromuscolar dysplasia (FMD) has been correlated to SCAD occurrence.The prevalence FMD among SCD victims are unclear. Myocardial infarction with non-obstructed arteries (MINOCA) could represent SCD. Since clinically manifest as myocardial infarction, expert consensus documents have often considered subtype MINOCA (1,2), but studies which address direct comparison between these two conditions lacking. Purpose To assess characteristics decedents and/or found at autopsy, compare their clinical pathological profile victims. Methods We reviewed database 5325 consecutive cases SCDs referred our pathology center 1994 July 2017. Results identified 21 (0.4%) 37 (0.7%) (3), whereas was only in 2 (0.04%). were females 81%, versus 38% (p=0.02). No signs Necrotic myocardium the totality 8 (38%) (p<0.001). Pre-mortem symptoms present 100% 49% (p<0.001); illicit drug use reported none 46% (p=0.001). Conclusions rare Compared MINOCA, more frequently females, always experienced pre-mortem no habit substances abuse. At autopsy not shows different profile. should be MINOCA.
